Aboriginal Health Coaching & Wulumbara
Aim of the Service
Health Coaching focuses on improving the health and wellbeing of people living with chronic health conditions. It is ideal for people who feel ‘lost’ in the health care system and would like some guidance, or for those who know what they need to do to improve their health but are not sure where to start.
The role of the Aboriginal health coach can support you with group exercise classes in the Wulumbarra program.

- Indigenous people 16 years and above
How to get an appointment
Individuals can self-refer through our website or can phone our intake team. We also accept referrals from GPs and other health professionals via the PCC website, medical objects or emailed to intake@primarycareconnect.com.au
