Your Health Coach will support you in all your health care needs, such as Diabetes, Cardiovascular Disease, Chronic Obstructive Pulmonary Disease, Hypertension and much more.
Eligibility
Chronic Obstructive Pulmonary Disease (COPD) such as Emphysema or
chronic Bronchitis
Cardiovascular disease
(history of heart attack, coronary bypass grafts or stents)
Chronic heart failure
Stroke or transient
ischaemic attack – ‘mini stroke’
Pre-diabetes and diabetes
High blood pressure and/or
high cholesterol
