Diabetes Education
Diabetes is a serious and chronic condition, which can affect the entire body. While there is currently no cure, you can live an enjoyable life by learning about the condition and managing it effectively.
Primary Care Connect provides Diabetes Education and support for clients diagnosed with pre-diabetes and diabetes, their families, and support people. Areas of support that our Diabetes Educator can provide are:
- Diabetes Education for client self-management - to continue leading a fulfilling life
- Reduce or prevent the need for hospitalisation by early recognition of problem areas, and arranging for appropriate referral and/or treatment
- Annual cycle of care review
- Provide advice, encouragement and support for clients, families and support people
- Provide advocacy, support and information for the client with their GP and/or service providers

The Australian Type 2 Diabetes Risk Assessment Tool was developed by the Baker IDI Heart and Diabetes Institute on behalf of the Australian, state and territory governments as part of the COAG initiative to reduce the risk of type 2 diabetes.
