The Regional Health Navigator Program provides support for you to find the right care. It helps people living with chronic conditions, including Diabetes, COPD, and heart disease, get the right care, in the right place, at the right time.

For people living with:

Eligibility

Have a diagnosis of diabetes, COPD, and/or chronic heart disease
18 years old or over
Live in Greater Shepparton, Moira, Mitchell, Murrindindi or Strathbogie shires
